‘Constipation’ or Obstructed Defaecation

A clear distinction needs to be made between an inappropriate evacuatory effort resulting in the anus tightening rather than letting go when trying to pass stool and an inability to pass stool due to there being no stool to pass (constipation).

This results in the feeling of NEEDING to go, but being unable to. This often necessitates getting up, walking around for 20 minutes or so, and then trying again… (referred to as evacuatory attempts).

Not SLOW COLONIC TRANSIT which is True Constipation and refers to slow movement of faecal matter through the gut, resulting in hard stools.

Treatments include:

Physio:

pelvic floor exercises
muscle re-education and rehabilitation
biofeedback, including electromyography (EMG)
nerve and muscle stimulation (NMS)
defaecatory technique (getting things moving)
advice regarding increased fluid intake and appropriate exercise

Dietary:
of major significance – consult dietician
Medical:
stool softeners
a variety of laxatives - stimulant or bulk-forming
other (consult Gastroenterologist or GP)
Surgical:
a variety of surgical procedures (consult Colorectal Surgeon)
